History and preoperative clinical and laboratory data were retrospectically investigated in 862 bitches with pyometra, and associations between the variables and peritonitis, prolonged post-operative hospitalization and mortality in surgically treated dogs were analyzed.
Variables of possible diagnostic value which have not been described previously were discovered, and several new prognostic markers for peritonitis, prolonged post-operative hospitalization and mortality were identified.
In clinical practice the results of this study may facilitate diagnosis and an earlier identification of bitches at risk for complications, which in turn will aid in the optimization of treatments and subsequently improved prognosis. |

Pyometra är en av de vanligaste sjukdomarna hos intakta tikar. Sjukdomen har ibland dödlig utgång och sjukdomsförloppet kan kompliceras av peritonit och systemiska effekter av infektion och inflammation vilket medför sämre prognos. Det är därför angeläget att identifiera tikar i riskzonen så tidigt som möjligt i sjukdomsförloppet. Syftet med denna studie var att identifiera prognostiska markörer för peritonit, morbiditet och mortalitet vid pyometra hos hund, samt att kartlägga förekomst av vanliga fynd som kan underlätta vid diagnostik av sjukdomen. I denna retrospektiva studie av 862 tikar med pyometra noterades förekomst av ett flertal preoperativa anamnestiska, kliniska och laborativa fynd, och associationer mellan dessa variabler och förekomsten av peritonit, förlängd postoperativ djursjukhusvistelse respektive mortalitet vid kirurgisk behandling av pyometra undersöktes. Kirurgisk behandling av pyometra utfördes på 773 av tikarna. Av dessa hade 14,2 % peritonit, 7,9 % förlängd postoperativ djursjukhusvistelse definierat som tre dygn eller mer, och mortaliteten var 2,1 %. Ett flertal prognostiska markörer för peritonit vid pyometra identifierades i denna studie, varav förekomst av diarré, livmoderförstoring, toxiska neutrofiler, normal/hög hematokrit och stegrat alkalisk fosfatas (ALP), laktat och CRP inte tidigare rapporterats vara associerade med förekomst av peritonit. Nya prognostiska markörer för förlängd postoperativ djursjukhusvistelse efter kirurgisk behandling som identifierades var förekomst av toxiska neutrofiler och stegrat ALP. Ett flertal prognostiska markörer för mortalitet vid pyometra kunde identifieras vilka inkluderade nedsatt allmäntillstånd, avvikande slemhinnefärg, avvikande slemhinnefärg, kräkning, diarré, dehydrering, leukopeni, neutropeni, avvikande glukos, förhöjt kreatinin och förhöjt laktat. Förekomst av stavkärniga neutrofiler och förhöjt C-reaktivt protein (CRP) hos cirka 70 %, förekomst av toxiska neutrofiler och förhöjt laktat hos 25–30 % och livmoderförstoring hos nära 90 % av undersökta tikar var nya diagnostiska fynd som gjordes i studien.
